UPDATE as of 7/9: There are only three forms required for each youth. The Bethel Travel Form is to be filled out by only one leader. And our ServantLife project does NOT require additional release forms!
Not to be overly OCD (as Lilly likes to say), but do we know our congregational ID for the ELCA medical form? Or do we just write in Bethel?

ReplyDeleteBy the way, 'overly OCD' is redundant, because OCD is already describing an extreme or maximum state.
I just wanted to make that totally crystal clear, not that I know much about OCD... just saying... ;-)
Is someone submitting the new insurance form on behalf of the whole group? Looks like it just needs to be one form for the group.
